A British Citizen is someone who is permitted to remain in the UK indefinitely without any visa restrictions. A British National Overseas is someone who is born in an overseas territory.

Do British Overseas Nationals automatically get a British Passport?

No, they must apply under a BNO Visa to enter the UK. After a qualifying period has been completed, they may become eligible to apply for indefinite leave to remain. Upon achieving Indefinite Leave to Remain they can then apply to become a British Citizen and obtain a British Passport.

Do I have access to Free NHS Healthcare?

No, you will have to pay an Immigration Health Care Surcharge in order to access any medical care within the UK. If you intend to obtain private health care this does not make you exempt from the Immigration Health Care Surcharge as it is a requirement of you making your Visa application.
